What You’ll Do as a Home Health RN
  • Deliver high-quality, patient-centered care in the home setting

  • Maintain primary responsibility for assigned caseloads, including assessment, care planning, implementation, and evaluation

  • Conduct initial and ongoing nursing assessments addressing physical, psychosocial, and environmental needs

  • Make regular patient visits and re-evaluate care needs throughout the episode

  • Communicate significant findings and changes in patient condition to physicians, supervisors, and care team members

  • Update and revise plans of care as patient needs evolve

  • Ensure all documentation meets clinical, regulatory, and HIPAA standards

  • Collaborate closely with interdisciplinary team members to support optimal patient outcomes

✅ What We’re Looking For
  • Current, unrestricted RN license (state of practice)

  • 1+ year of clinical nursing experience

  • Willingness to travel within the branch coverage area (approximately 50%)

  • Valid driver’s license, dependable vehicle, and current auto insurance

  • Associate or Bachelor’s Degree in Nursing (preferred)

  • Home Health or Hospice experience (preferred)

  • Experience with HomeCare HomeBase (preferred)

This position reports to the Clinical Manager or Clinical Supervisor.
