POSITION SUMMARY:
At Stony Brook Southampton Hospital, the ED RNCM must have acute care or transitional care experience. The EDCM will assess patients in the ED for level of care, perform assessments and discharge planning as indicated. This CM will also screen for patients who may be appropriate for transfer to SBSH from a generate list and reach out to SBUH as indicated in an attempt to proactively transfer patients to SBSH. This RN must have good clinical and communication skills to aid in the coordination of level of care, discharge planning, transfer care and facilitate communication between all involved parties
RESPONSIBILITIES:
- Prior experience with Care Management in either acute or transitional care.
- Understands levels of care including Inpatient, Observation, Emergency Room, and Outpatient.
- Understands insurance coverage and authorization.
- Effectively works with the interdisciplinary team regarding transitions of care including Case Managers, Social Workers, Pharmacists, Medical staff, Nursing, Physical Therapist, etc.
- Understands the bed board and bed capacity management.
- Communicates daily with nursing to understand staffing capabilities.
- Coordinates communication between the ED Physicians, medical team, nursing and case management.
- Serves as an advocate and liaison for patients in the ED as well as possible transfers.
- Tracks and trends data with analysis of information.
- Actively involved in performance improvement activities including program development and improvement.
- Adheres to all State and Federal requirements, RN license, JACHO, CMS and DOH regulations.
- Maintain high level customer service.
REQUIRED EXPERIENCE AND QUALIFICATIONS:
- Bachelor's degree in nursing with 3-5 years acute care experience.
- NY RN License.
- Acute hospital experience or transitional experience.
- Demonstrate knowledge of discharge planning and length of stay.
- Courteous, professional, focus and good listening skills.
- Computer literate.
PREFERRED EXPERIENCE AND QUALIFICATIONS:
- Master's degree.
- Bilingual.
- Certified in Case Management, UM, Quality, Risk or MCG.
- Experience with post-acute care coordination, including acute rehab/skilled nursing.
- ICU or ED experience.
- Home Care experience.
- Telehealth experience.
As part of employment at Southampton Hospital, there may be occasions when travel to other locations is needed to support operational needs.

Anticipated Pay Range:
The starting salary range (or hiring range) for this position has been established based on relevant experience.
The above salary range (or hiring range) represents SBSH's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Prior to start date, the selected candidate must meet the following requirements:
Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Southampton Hospital's Employee Health Services*
- Complete electronic reference check with a minimum of three (3) professional references.
- Successfully complete a 4 panel drug screen*
- Meet Regulatory Requirements for pre-employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).
Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.
*The hiring department will be responsible for any fee incurred for examination.
